Medical Alert Customer Application

Waunakee Utilities wants to work with you to help ensure you have adequate plans in place in the event of an extended power outage. Please be aware that being on the Critical Needs List does not guarantee uninterrupted electric service.

While we work hard to minimize outages, there are situations beyond our control where electric service may be interrupted. During the summer months, the likelihood of outages may increase due to storms and periods of excessive heat.

To be added to or remain on the Critical Needs List, please complete the application and return it along with a signed statement from your physician.

Medical Emergency Protection

You may qualify for up to 21 days of protection from electric service shut-off due to nonpayment if your household includes:

  • An infant or young child

  • An elderly person

  • Someone with developmental or mental disabilities

  • Someone who is seriously ill

  • Someone who uses a life-support system

To receive this protection, you must submit a Medical Emergency Protection Statement completed by a licensed physician, public health official, social services official, or law enforcement official. The statement must describe the emergency condition and specify the time period during which disconnection would worsen the situation.

How to Apply for the Extension

  • Obtain the Form: Contact Waunakee Utilities to request a medical certification form.
  • Get Certification: Have a licensed Wisconsin physician, public health official, or social services official fill out the form certifying the medical emergency. The form must specify the nature of the emergency and how the lack of service will aggravate it.
  • Submit Promptly: Submit the signed form to the utility as soon as possible, ideally within 3 business days of a shut-off notice, to ensure protection.
  • Payment Plan: While the extension is in place, you must work with the utility to establish a reasonable payment arrangement.
