Sign Up For New Residential Service

Welcome to Waunakee Utilities! We're happy to be your new utility provider. Please submit the form below to establish service at your new residence. We are available for connects and disconnects Monday through Friday. We do require at least a 24-hour notice.

Customer Information

We are available for new service connections, Monday through Friday.
A photo ID is required to provide new service. Scan or take photo of your current ID and upload the image here.
Files must be less than 5 MB.
Allowed file types: gif jpg jpeg png bmp eps pdf.

New Address

If you own your home, provide the name of your closing title company here.

Spouse or Roommate Information

Previous Address

If you owned your previous home, provide the name of your closing title company here.
If a current Waunakee Utilities customer, date you want service ended at previous address, Monday through Friday.
